As a Regional Administrator, you can delete forms that are no longer used. Deleted forms are removed from the solution and no longer available for administrators or users.

Note: Forms created by Juvare cannot be deleted.

To delete a form

  1. On the upper right, click System Settings.
  2. On the left, under Other Settings, click Patient Forms. The Forms Configuration page opens.
  3. Locate the form you want to delete. On that row, in the Actions menu, click Delete. A Warning window opens asking you to confirm the deletion.
  4. Click Yes. The Forms Configuration page appears and your form is no longer in the list.
