This Confluence Page provides the page is provided for the Illinois Department of Public Health (IDPH) Vaccination Tracking project. It is designed to offer links, documents and training videos for the EMtrack Solution for the Illinois Department of Public Healthused by the IDPH.

Enrollment Process:

For Local Illinois Health Departments in utilizing the Illinois EMTrack system to enable scheduling and support operations for mass immunization in Phase 1B and Phase 1C of your counties vaccination plan.  Please follow the instructions and link below to enroll into the system.

1. Go to: https://forms.juvare.com/forms/52ab18e3-04ae-4eeb-b3f5-75a6bcc8dca9
2. Enter the Password: IDPH&EMTrack2020
3. Complete the form.

Note: This step will need to be repeated for each individual site. You may complete this process multiple times if you have multiple sites or multiple 1-day Point of Distribution (POD) operations.  During enrollment, you will have various registration/schedule options to fit your needs.

After Enrollment:

Once the enrollment form has been submitted and the vaccination site has been created in EMTrack you will receive confirmation from Juvare. Use the following links and documents below to access training material and the EMTrack Solution.
