A provider facility is a standing facility, which can be a hospital, long-term care facility, or shelter.
Labels
Labels allow you to categorize your provider facilities by the type of facility, services provided, and more. Refer to Facility Labels for more information.

Multi-region Response
You can designate a provider specifically for use during multi-regional incidents. Typically, this provider facility is designated as an Aerial Point of Embarkation or Debarkation (APOE/APOD). For this point of transfer, be certain to select the Regional Embarkation/Debarkation Point check box. In addition, consider specifying the following:
Field | Description |
---|---|
Agency Number | Provider number your State assigned to your facility/agency |
Labels* | Type of facility and/or type of services your facility/agency provides Examples: Long-term Care, Public Health, Hospital |
*Labels can be used to locate the appropriate providers when adding or editing an incident. |
Tip: Consider adding the Agency Number and Labels to each of the provider facilities in your region to facilitate patient tracking and data sharing.
