This page is provided for the Illinois Department of Public Health (IDPH) Vaccination Tracking program. It is designed to offer links, documents, and training videos for people using EMtrack in conjunction with the IDPH.

Enrollment Process

The enrollment process will be handled by local Health Departments, utilizing the Illinois EMTrack solution to enable scheduling and support operations for mass immunization in Phase 1B and Phase 1C of your county's vaccination plan.   Please Please follow the link and instructions below to enroll in the solution.

1. Go to: https://forms.juvare.com/forms/52ab18e3-04ae-4eeb-b3f5-75a6bcc8dca9
2. Enter the Password: IDPH&EMTrack2020
3. Complete the form.

Note: This step needs to be performed for each individual site. You may complete this process multiple times if you have multiple sites or multiple 1one-day Point of Distribution (POD) operations. During enrollment, you will have various registration/schedule options to fit your needs.

After Enrollment

Once After the enrollment form has been is submitted and the vaccination site has been created in EMTrack, you will receive a confirmation email from Juvare. Use Please use the following links and documents below to access training material and the EMTrack Solutionsolution and training materials.
