This page is provided for the Illinois Department of Public Health (IDPH) Vaccination Distribution program. It is designed to offer links, documents, and training videos for people using EMTrack in conjunction with the IDPH.
Enrollment Process
The enrollment process will be handled by local Health Departments, utilizing the Illinois EMTrack solution to enable scheduling and support operations for mass immunization in Phase 1B and Phase 1C of your county's vaccination plan. Please follow the link and instructions below to enroll in the solution.

- Enter the password provided to you by IDPH.
- Complete the form.
Note: This step needs to be performed for each individual site. You may complete this process multiple times if you have multiple sites or multiple one-day Point of Distribution (POD) operations. During enrollment, you will have various registration/schedule options to fit your needs.
After Enrollment
After the enrollment form is submitted and the vaccination site has been created in EMTrack, you will receive a confirmation email from Juvare. Please use the following links to access the EMTrack solution and training materials.
